ORIGINAL FORMAT	Black and white photograph, 9 1/2" by 7 1/2" ; slight fraying of lower right corner ; small chunk missing approximately 2" above the lower right corner ; the number "615", slightly smudged is written in the upper middle ; a faint impression of a previous number can be seen below the "615".  The picture is glued to a woven material background that extends beyond the left of the picture.  This material has several puncture holes (for insertion into a binder) (615)
